In one creator session, a player loaded 14 The Apex expecting Heavy Armor and rolled Light Armor. The later explanation was that the piece he was actually chasing, Warlord's Golden, sits in the Light Armor band. Nobody was cheated by a hidden rule. The preview had said 95%, and 95% is not 100%. The mistake was treating a community threshold as a guarantee and then blaming the result.
After the roll: Equip, Enhance, Index, Rebirth
Once a piece is in the Backpack there are four systems it can feed, and each one is optional and independent.

Equip is the one that has no cost and no downside. Three sources describe the same behavior: a forged piece lands in the Backpack and sits there until you open the Weapon tab and press Equip. A strictly better helmet sitting in a bag changes your survival by exactly nothing.
Enhance is the hub's single station, in the middle of the lobby, and it is the only place in the game that takes weapons, helmets and chest pieces. Index is the red book in the bottom-left corner of your screen, and it has a separate Helmet tab and a separate Armor tab. An entry you do not own renders as a black silhouette with ???, so the book doubles as the list of armor you still owe yourself. Milestone rewards are Power Boost +5% and bag capacity +2, and every source that has seen one agrees on the same trap: you have to click it yourself. Uncollected entries cost you nothing but the click, and the reward is permanent.

Rebirth is the reason a bad roll is still worth keeping. The in-game warning names one stat, and it names only Power. Weapons, helmets, chest pieces, ores, Index milestones, permanent upgrades, Max Ore Count and your saved Coins all survive, which five independent source families agree on. So a 14-ore Heavy roll that lands at 95% and misses is a piece you take into the next cycle, not a mistake you repeat.
Power is the only thing the red warning on the rebirth button says it deletes. Everything else on this page persists through the reset, including Index milestones you have already claimed. Community reads call Cursed Void a helmet and Void Overlord a helmet plus armor, with a Power range somewhere between +56% and +83% for the latter. That range comes from a single source and has never been reproduced. If the health bar is not moving, the weapon and the training floor are the problem, and a better helmet will not change that. Power is the floor under your damage and your weapon multiplies it.
