Two independent sources agree on the split, statue versus book, and both agree on the six skill ranks. Everything downstream of that is guesswork.

The four tabs, locked entries, and sort order
Weapon, Helmet, Armor and Ore. That split comes from a single source, so treat Helmet being separate from Armor as likely but not certain; a second source groups the same content as weapons, armor sets and raw minerals without breaking helmets out. Either way a helmet does not tick your Armor tab, and the tabs track independently.

There is no published level count, no XP requirement per level, and no ladder between Lv.4 and Lv.12 or above it. The two are not adjacent steps in a known sequence, so you cannot interpolate a curve from them the way you can from the rebirth gates.

Every source that has watched someone hit a milestone repeats the same warning: the bonus sits there unapplied until you press the claim button, and the book will show it as available indefinitely without giving you anything. Make claiming part of your run loop, and check before you rebirth, since a reset is a bad moment to find one unclaimed.
Claimed milestones are permanent and survive rebirth, confirmed by three sources. They are also the only progression completely immune to the Power wipe, which makes them the cheapest long-term value in a run. The differences may be two different tiers rather than a contradiction, one site naming a joke-flavored label for a lesser reward. Or one of the two may be wrong, and neither has been withdrawn, so read it off your own book. Max Ore Count starts at 4 and caps at 24 through a separate upgrade station, and one account reports Index rewards pushing the bag past that cap, which would make the two sources additive.
The Skill Index statue
Confirmed by two sources as a lobby statue, separate from the red book, recording every skill you have owned, with skills ranked across six grades: D, C, B, A, S and SS. No tier above SS has ever been reported, and no source explains whether the statue gives XP, milestones, or anything else at all.
